Appointment
The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
You will need to bring the following items to your appointment:
Driver's license or photo ID
Paperwork that you received from us or that you printed from our website
Any current x-rays (Panoramic within 3 years and/or Bitewings within 1 year)
Dental insurance information
Please arrive 15 minutes before your scheduled appointment.
If a patient is a minor (under 18 years of age), a parent or legal guardian must be present at the first appointment.
CANCELLATIONS ARE REQUIRED AT LEAST 24 HOURS PRIOR TO YOUR APPOINTMENT. A $25.00 NO SHOW FEE MAY BE ASSESSED.

Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
Payment Options
For your convenience, we accept cash, personal checks, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
